How to Create a Forever 21 Credit Card User Account

To get started, navigate to the Forever 21’s website and press “Forever 21 Credit Card".

If you start from the Comenity website for cardholders, find the black “Apply” button on the card information box. You will be redirected to the Forever 21’s website as well.

One more time, select “Apply”.

At this point, the website will request your phone number, the last four digits of your SSN, and ZIP code. You can skip this step by choosing the option "Apply Without Pre-fill" below.

If you decide to complete the optional form, click “Continue”. You will receive a verification code via SMS. Enter the code and click “Continue” again.

The website will request your personal information: legal name, address, and future credit card phone number. You will also need the trailing digits of your identification document. Fill every field as instructed.

If you would prefer a Visa credit card, which allows its holder to pay and gain rewards anywhere, check the box below.

Following this, click “Next”.

Input some more information about yourself. That is your email, date of birth, and annual income. Again click “Next”.

If you would like to authorize someone else to use your credit card for purchases, you can do it at this step. Input their name, date of birth, and relationship to you. Alternatively, check “No” to keep the card only for yourself. Click “Next” to proceed.

Tick the box to confirm that you have read and agree to the terms and conditions. Complete your application using the “Submit” button.

Now all you have to do is wait. The bank should inform you of its decision regarding the credit card application within two weeks.
